Your first visit to a Podiatrist can be overwhelming, but don’t worry, our friendly staff and doctor will help make it a pleasure. For your first visit, bring your insurance card, along with a referral from your primary care doctor, if your insurance card requires it.
 
Make sure you get this done before the appointments. Along with this, you would need to fill out and bring your completed new patient information form, or you can even fax it to our office at (920) 882-9544.
